Every application undergoes a thorough review, including a site visit, if eligible, to assess project scope and feasibility. Please note: a site visit does not confirm project acceptance. Funding for projects is reviewed through CLTS/IRIS, Logan’s Heart and Smiles, Custodian, or a combination of these options.
Eligibility requirements
- The child must be under the age of 21, living in Wisconsin, and must have 50% residency at the location of desired home modification.
- Work done to residence must enhance the ability of recipient to move through, in/out of the home, ensure safety, or be for therapeutic use.
- Approval from Landlord, HOA, or similar must be provided in writing prior to construction if required.
- Recipients to receive services on a first come, first serve basis. Priority is given to those with greatest need/risk. Weather also plays an important role in determining when projects can be completed.
