Use the button below to apply for assistance from Logan’s Heart and Smiles. Please review the eligibilty requirements prior to filling out the application. Every application will be reviewed for funding through CLTS/IRIS, Logan’s, Custodian, or a combination.
Eligibility requirements
- The child, under the age of 21, must have 50% residency at the location of desired home modification.
- Work done to residence must enhance the ability of recipient to move through, in/out of the home, ensure safety, or be for therapeutic use.
- Approval from Landlord, HOA, or similar must be provided in writing prior to construction if required.
- Recipients to receive services on a first come, first serve basis. Priority is given to those with greatest need/risk. Weather also plays an important role in determining when projects can be completed.
- Recipients are limited to one project per 12 months to allow us to help as many children in need.
